A dermatologist who has referred five post-Mohs reconstructions here wrote that the operative note she receives back is more detailed than the ones she gets from the academic centre, and that the patient always knows what happened to them and why. Both halves of that are deliberate. The operative note is written the day of surgery and states the defect, the reconstructive choice, the alternatives considered and why they were not used, because a referring clinician managing that patient for the next decade needs the reasoning and not only the procedure code. The patient gets their own version in plain language before they leave. This is not a marketing decision and it costs about twenty minutes per case. It is, on the evidence of where our surgical referrals actually come from, the single most effective thing the practice does that nobody outside it ever sees.

She wrote that Adaeze asked what a Tuesday looked like instead of handing over a meal plan she was never going to follow. That is the method, described more clearly than we have managed in six years of trying to explain it. We fixed breakfast first and left the rest of the week alone for a month.

We are not counting that as a loss. A consult that ends in "come back in six months" is a plan, and most of those people do come back.
