Instruments are counted before an operation begins, counted again before closing, and counted a third time at the end. It is not a Rowan innovation, it is standard operating room practice, and we mention it because patients almost never hear about the parts of surgical care that are deliberately dull. Most of what makes an operating day safe is repetition of things that have never gone wrong. That is the intended outcome of a small schedule.

When a patient sends us her sister, we are being handed a relationship that predates us and will outlast us, and the cost of a disappointing outcome is no longer one person’s. It is a marketing word, invented to make a serious operation sound like an errand, and it describes the patient rather than the surgery. What we perform is an individually planned combination of procedures, most often an abdominal wall repair with a breast procedure, chosen for one body and one recovery. There is a version of neuromodulator treatment that removes movement entirely, and it is not the one we practice. Sloane Holloway, RN, BSN, treats to soften rather than to erase, which means the first appointment is often deliberately conservative and the second one, two weeks later, adds only what the face turned out to need. It is slower. It costs a return visit. It is also the reason people tell us they cannot tell what changed. Bruising and temporary asymmetry are possible with any injection, and the effect wears off over three to four months.

The single instruction our surgical patients most often underestimate is sun protection over a healing scar. A new scar is more likely to darken with ultraviolet exposure for the better part of a year, and once it does, that pigment is slow to leave. This is why we ask for a physical sunscreen over the site daily, hats in June, and patience through a Tennessee summer. It is the least interesting part of aftercare and it changes the twelve-month appearance more than almost anything else we ask for.
